Volunteer Forms

Parents wishing to chaperone regularily throughout the year for band related events should have the following items completed and on file with the Highwood Office Administrator.

1). Criminal Record Check with Vulnerable Sector Check (obtained through local RCMP free of charge)

2). FSD38 Volunteer Registration Form

3). FSD38 Volunteer Offence Declaration Form

4). FSD38 Volunteer Confidentiality Form
